Abstract Many of the adolescents which are in the care of Youth Protection Services have mental health problems and disorders due to developmental trauma inflicted in close relationships. They usually have a weak mentalizing ability, which is expressed through problems perceiving and understanding their own and others condition and often misinterpret the intentions of others. They also struggle with affect regulation and social relationships and are later overrepresented as drug addicts and patients with serious mental health disorders. This thesis aims to investigate how mentalization as an approach can be a useful tool in environmental therapeutic treatment to help these young people. The thesis problem statement thus reads as follows: What significance can a mentalization-based approach have in environmental therapeutic change work with developmentally traumatized young people in Youth Protection Services? In the thesis we use theories that shed light on the connection between mentalization and other important concepts and approaches, and which show its complexity and significance. We have thus chosen theories about environmental therapy, relationships, TBO, attachment, developmental trauma, mentalization and mentalization-based therapy. In the chapter about research, we include relevant studies on the use of mentalization as a therapeutic treatment of young patients with socio-emotional difficulties. We also include a critical view of evidence-based knowledge in social work. In the discussion section, we concentrate on the following: First, we discuss how safe milieu therapeutic relationships affect mentalizing ability in adolescents. Here we highlight acknowledgement, respect, empathy, control over one's own emotions and not-knowing stance as important qualities in the milieu therapist to create safe relationships and stimulate the development of adolescent's mentalizing ability through them. Second, we seek knowledge about what the mentalization-based approach has to offer in current practice in Youth Protection Services. Here we compare mentalization-based approach with TBO to find out which of the two can work more effectively in milieu therapeutic treatment. Third, we discuss the challenges that may arise in the use of a mentalization-based approach in Youth Protection Services. We conclude that the milieu therapeutic approach, despite some limitations, can be a promising treatment for developmentally traumatized adolescents Youth Protection Services.en_US
